  • You may use the English or Hebrew date of birth/death (if you need help with this, please call the office)

*For every plaque sold, you will receive a free leaf for the Tree of Life on the wall outside our Memorial Room. This can be used to honor a loved one; remember a memory; congratulate a graduation; or mark a simcha. Leaves on the Tree of Life are used to mark many different occasions.
